WHAT IS A HOST HOME?

A Family or individual who opens their homes and hearts to individuals with intellectual/developmental disabilities (IDD) by providing them with services and support while owning their own home-based business. Becoming a host home provider and bringing an IDD individual to your family will likely be one of the most rewarding experiences you will ever have.

WHAT SERVICES DOES A HOST HOME PROVIDE?

Services will depend on needs and level of support of the individual, but these are some of services a host home may provide:
• Supervision
• Personal Care
• Community involvement through engaging activities
• Learning how to cook
• Managing money
• Coping skills
• Transportation to and from day programs & medical appointments

WHAT ARE THE RATES?

$1,500 – $5,000/month depending on the needs and level of support of the individual (Host homes are independent contractors).
• Levels of care range from 1- low needs to 6 – high needs.
• Host home families can serve one to 3 people in their homes.
• Host home providers also receive a transportation allowance.
• Payments continue when clients are not in service (hospital stays, visits to family, etc.)

WHO WILL HELP SUPPORT HOST HOME PROVIDERS?
• The host home manager will walk alongside you during transition and will be there when you have questions.
• Community Options, Inc. provides 24 hour on-call for all host homes.
• We will serve as an advocate for the person(s) you serve and work with you to meet their needs.

WHAT ABOUT ADMINISTRATIVE SERVICES?

Community Options, Inc. provides:
• Comprehensive training such as First Aid/CPR & Medication certification, etc.
• Monitoring for compliance & various reporting
• Medicaid billing based on monthly invoices
• Pharmacy liaison and certain nursing services

REQUIREMENTS TO BE A HOST HOME PROVIDER?
• Must be at least 18 years old
• Must be able to pass all background & motor vehicle checks.
• Must have reliable transportation
• have a separate bedroom in their home and can pass HUD standards
